Fast Track Therapy (ABN 71 683 394 309) provides occupational therapy services. This policy explains how we collect, hold, use and disclose personal information and health information, and how you can access, correct or complain about it. We handle your information under the Privacy Act 1988 (Cth), the Australian Privacy Principles, and the health records legislation of the state or territory in which the service is delivered.
The information we collect
Personal information includes name, date of birth, contact details, emergency contact, NDIS participant number, funding arrangement, and the details of a support coordinator, recovery coach or other person making a referral.
Health information is sensitive information under the Privacy Act. For our clients this can include assessment findings, clinical notes and observations, diagnoses and medical history, functional capacity and daily living information, equipment and assistive technology prescriptions, home environment details, medications, referrals and reports to and from other practitioners, care and support plans, appointment records and consultation metadata, and Medicare or private health fund claim details.
Referrals made on behalf of someone else
Where a support coordinator, recovery coach, family member or other professional submits a referral, we rely on that person having the client's consent to share the client's details with us. We ask for only the minimum information needed to make contact and confirm the service is suitable. Clinical records and detailed histories should not be sent through the website form; we collect what is clinically necessary directly from the client or with their consent.
Dealing with us anonymously
Due to the nature of the health services we provide, it is generally impracticable for us to deal with individuals who have not identified themselves. Accurate identification is required to ensure the safety and continuity of clinical care, to comply with our legal and professional obligations under applicable health legislation, and to meet the requirements of Medicare and private health insurance billing. You may make a general enquiry without identifying yourself.
Unsolicited information
From time to time, we may receive personal or health information that we did not solicit. Where we receive unsolicited personal information, we will promptly assess whether that information is of a kind we could have collected under our standard collection practices. If it is not, and the law permits, we will destroy or de-identify it.
How we use your information
We use personal and health information to deliver occupational therapy services, to assess whether a referral is suitable, to prepare assessments and reports, to communicate with you and, with consent, with your support team, to arrange and confirm appointments, to invoice and claim, and to meet our legal, professional and insurance obligations.
Direct marketing
We do not use health information for direct marketing. We will only send marketing communications where you have expressly opted in, and every such message includes an unsubscribe option. Submitting a referral or enquiry does not opt you in to marketing.
Who we disclose information to
With your consent, we may disclose information to your support coordinator or recovery coach, treating practitioners, equipment suppliers and builders involved in a prescription or modification, and funding bodies including the NDIA, insurers and Medicare. We also use service providers for practice management, secure file storage, email and appointment reminders, who are bound to protect the information.
We may disclose information without consent only where the law requires or permits it, including where there is a serious and imminent threat to life, health or safety, mandatory reporting obligations, a court order or subpoena, or a notification under the Health Practitioner Regulation National Law.
Overseas disclosure
Some of the software we use to run the practice stores data on servers located outside Australia. Where that occurs we take reasonable steps to ensure the overseas recipient handles the information consistently with the Australian Privacy Principles. Cookies and website tracking
This website uses cookies and similar technologies for analytics and advertising measurement. Because browsing a health service website can reveal sensitive interests, we do not use health information for advertising, and we do not build advertising audiences from the contents of a referral. You can block or delete cookies through your browser settings, which will not affect your ability to contact us by phone or email.
Storage, security and data breaches
Records are held in secure electronic systems with access limited to those who need it to deliver or administer your care. If a data breach occurs that is likely to result in serious harm, we will notify the Office of the Australian Information Commissioner and affected individuals as required under the Notifiable Data Breaches scheme in Part IIIC of the Privacy Act.
How long we keep records
Clinical records are retained for the period required by the health records legislation applicable in the state or territory where the service was delivered, running from the date of the last entry in the record, or, for a client who was under 18, until they reach the age required by that legislation. Records are then securely destroyed or de-identified.
Access and correction
You may request access to the personal and health information we hold about you, and ask us to correct it if it is inaccurate, out of date or incomplete. Contact us in writing at [email protected]. We will respond within a reasonable period. Access may be limited in the narrow circumstances the Privacy Act allows, and we will explain our reasons in writing if that applies.
My Health Record
We do not currently upload documents to My Health Record. If this changes, this policy will be updated and consent will be sought before any upload.

If you have a concern about how we have handled your information, contact us first at [email protected] so we can try to resolve it. You can also complain to the Office of the Australian Information Commissioner at oaic.gov.au, or to the health complaints body in your state or territory.
