Being a small local provider means direct communication and fast sign up times. Appointments happen at the client's home, workplace or another community setting that suits them, with telehealth used where it is appropriate.

Not every participant needs a full 10 hour assessment. Sending the right one first saves a plan, and saves you re-referring later.

A brief FCA may suit
- NDIS access evidence, documenting baseline functional capacity for an initial application
- Single issue reviews, such as a minor support hour adjustment or standard low cost assistive technology
- Situations needing a focused process with reduced clinical and report writing overhead
A full FCA is required for
- Multi layered physical, cognitive or neurodivergent support requirements
- Home and living requests, including SDA, SIL and ILO clinical justification
- Customised equipment prescriptions such as power wheelchairs, or major home modifications

- NDIS: Travel limit 30 minutes each way, as per the applicable NDIS Pricing Arrangements and Price Limits.
- iCare or Lifetime Supports: Travel limit 30 minutes total. Fees remain subject to the approved arrangements and service agreement.
- Private payment: Includes GST. The call-out fee applies when the client is more than 5km from the closest clinician. A Medicare or private health rebate may be available depending on circumstances, and a gap fee still applies.

The part of a referral that costs you time is not the form, it is chasing the provider afterwards. Here is what happens, and when.

How do I refer a participant?
Use the referral form on this page, call Dylan directly, or email. Once we have the sign up details we need, the service agreement is sent within 24 hours and the participant is contacted within 48 hours.
What information do you need to get started?
The participant's contact details, NDIS number if available, funding information, goals, and any relevant reports or background. Please do not paste clinical records into the form. Dylan will collect what he needs on the call.
Who can you see?
Adults and adolescents aged 16 and over experiencing disability, injury, illness, or communication and functional difficulties. Fast Track Therapy does not hold a paediatric caseload.
Do you have capacity right now?
Capacity changes regularly. Run the referral check on this page or send the referral through, and Dylan will tell you his current availability when he calls.
Will I get updates?
Yes. Support coordinators are told when appointments are booked, and can be kept informed through the participant's journey, with the participant's consent.
Does the client need a GP referral?
No. Support coordinators, recovery coaches, allied health professionals and clients themselves can make contact directly. A GP referral is welcome where one already exists.
Why does the brief FCA say 'from' $969.95?
$969.95 covers 5 billable hours, plus travel where the appointment is in person. Scope and individual complexity change the final fee, which is agreed in writing in the service agreement before any work begins.
What areas do you cover in Perth?
Dylan covers the Fremantle catchment directly, roughly within 15km. Referrals from the wider Perth metropolitan area are welcome, and travel is confirmed when Dylan calls you back.
Is there a clinic the client attends?
This is a mobile service. Appointments happen at the client's home, workplace or another community setting that suits them. Telehealth is used where it is appropriate for the client.
Which funding arrangements do you work with?
NDIS participants across self managed, plan managed and NDIA managed funding, insurance schemes including icare and Lifetime Supports, and private clients who may be eligible for a partial Medicare rebate depending on their circumstances.
What happens if the participant needs to cancel?
Participants can cancel or reschedule online or by contacting us. SMS and email reminders are sent to help reduce missed appointments and avoid cancellation fees.
